New Patients and Appointments

Appointments:

To see Dr Kerry Mack you will need a letter of referral from either your General Practitioner or Specialist.  Referrals only last for a specific period of time a referral from a GP lasts twelve months, a referral from a specialist lasts three months

At your first appointment can you please ensure you bring your:

  • Letter of referral.
  • Medicare card, private health insurance details/card, concession card.
  • Information about past care and treatments.
  • List of current medications.

We will require you to fill out a short patient registration form to help us with basic contact and personal information. Our staff will work with you, respecting your privacy and confidentiality. Your first appointment generally lasts one hour.

Our Fees:

Our practice does not bulk bill.

Our fees will be explained to you when you make your first appointment. The cost of your appointment will vary depending on the length and nature of the session. A list of current fees will be provided on request including information on the Medicare rebate.

All fees are to be paid in full on the day of your appointment.

Cancellation of Appointments:

Please contact us if you are unable to attend an appointment, we have people on our waiting list to see Dr Mack.

You will receive an SMS appointment reminder asking you to confirm your attendance either YES or NO. Please respond ASAP to this message.

OR call us on 03-9468-0611 twenty-four hours before your appointment. If prior notification is not given, you will be charged a cancellation fee for the missed appointment. This fee does not attract a Medicare Rebate and must be paid in full before any further appointments will be made.
